Connie Jeane Hoffman, 64, passed away at her home on March 1, 2020.
Connie was born March 30, 1955 in Clarion, Iowa. She was educated in the Clarion Schools and later obtained further education and a certificate in nursing. She worked a variety of jobs in health care in the Des Moines area and often worked two jobs for many years. Most recently she worked for the Veterans Administration with homeless clients and for Lutheran Hospital. She was knowledgeable, hardworking, dedicated and passionate caregiver for her patients and clients.
She loved and cared very much for her family and those she helped in her work capacity.
She is survived by 4 children, J Hoffman in Alabama, Heather Hoffman, Cole Hoffman in Des Moines and Curtis Hoffman in Washington state; brother, Clair R. Cramer in West Des Moines; sister, Kim (Scott) Loggins in Cedar Rapids and Janica (Len) Williamson in Montana; grandchildren including, Xander, Tristan, Liam, Frank, AJ, Rex, Vina and Melissa; and several cousins, nephews and nieces. She was preceded in death by her parents, a brother, a sister and a son, Denny Hoffman.
Visitation will be held from 1 to 3 p.m., Sunday, March 8, 2020 at Hamilton’s Funeral Home, 605 Lyon Street, Des Moines. Her remains will be cremated.

Connie was a beloved Mobile Crisis worker that helped the Des Moines Police Dept. handle tough situations. Hardest working person I ever met. RIP.
