Hazel Van Horn

Hazel Phelps Van Horn, passed away Tuesday, July 15, 2008 at Vista Woods Care Center in Ottumwa.
Hazel was born June 11, 1919 to Lloyd and Wealthy Probasco on the family farm in Davis County. She married Gerald F. Van Horn on October 5, 1956. He passed away October, 8, 1964. Hazel graduated from Davis County School and from Gem City Business School in Quincy, IL. She worked for Federal Land Bank & National Farm Loan.
Hazel was a faithful member of Grant Park Christian Church for 52 years, served as CWF treasurer for over 20 years and was a member of the Stewardship Committee. She was also a member of the Daughters of Union Veterans of Civil War and served many years on the Polk County Election Board.
Hazel is survived by her sister, Helen Gooden of Ottumwa; five nephews, Larry (Pat) Gooden, Jerry (Karen) Gooden, Terry (Sandy) Gooden and Randy (Brenda Miller) Gooden, all of Ottumwa and Gene (Marjorie) Ellis of PA; and many great nephews and great-great nephews and nieces. She was preceded in death by her parents, husband and brother-in-law, LaVerne Gooden.
Funeral services will be held at 11 a.m. Saturday, July 19, 2008 at Hamilton’s Funeral Home, 605 Lyon St. with burial at Rising Sun Cemetery. Visitation will be held one hour prior to the service at the funeral home.
