Chak Phuong, 70, died Monday, June 9, 2008 at Mercy Hospital. Chak was born on November 7, 1937 in Palin, Cambodia.
He is survived by his children, Sophon Chak, Sambo Chak, Somaly Chak, John Chak, Chantha Bjerke, Savoeun Chak and Savak Chak; 19 grandchildren, and three great-grandchildren.
Funeral services will be held 2:00 p.m. Saturday, June 14, 2008 at Hamilton’s near Highland Memory Gardens, 121 NW 60th Ave., Des Moines. Public visitation will be 11:00 a.m. – 4:00 p.m. on Friday, June 13, 2008, as well as visitation on Saturday, June 14, from 9:00 a.m. until the service time, at the funeral home.

Chantha & Stuart - We were so sorry to hear about Chak's passing.
