Polk County General Assistance Application Process
If your family is applying for assistance from Polk County, the following forms will need to be completed and submitted: Polk County General Assistance Application and the Vital Statistics Form. Once Hamilton’s has received these completed forms from you, your funeral director will complete the request for assistance on your behalf.
